Background of the study
Transportation request forms are used to request transport services. Whether you run a shuttle service, taxicab company, or limousine rental, our free Transportation Request Forms are designed to help your company keep better track of clients. Transportation management systems play a central role in supply chains, affecting every part of the process from planning and procurement to logistics and lifecycle management. The broad and deep visibility afforded by a powerful system leads to more efficient transportation planning and execution, which results in higher customer satisfaction. That, in turn, leads to more sales, helping businesses grow. With such a dynamic global trade environment that we live and transact in, it is important to have a system that will allow you to successfully navigate complicated processes around trade policies and compliance.
Online transportation services are already well-known and well accepted in developed countries such as USA and Europe countries. In Indonesia, this type of service is now getting popular and many international companies were already started to expand their market in Indonesia. However, what is exactly online transportation service? Some studies use the term ride sharing to define online transportation services. It is called ride sharing due to the cars/vehicle used is owned by individual (private cars) which is then ‘shared’ with other (the customer) when delivering the services (Wallsten, 2015). Watanabe et al. (2016) explained that ride sharing is an on-demand services connecting passengers and vehicle owners (drivers) in real time using mobile technology. Online transportation service or ride sharing is now becoming a common means for people to fulfill their commuting needs.
Online transportation service is a part of m-commerce, customers or users can put order of products or services through internet without using PC (personal computer) (Clarke III, 2001). As the technology of mobile phone is getting more advanced, more people are using m-commerce and consequently more commercial services are provided by business people. Mobile commerce is attractive due to its unique characteristics. Clarke and Flaherty (2003) defines three characteristics of m-commerce which are ubiquity, convenience, localization and personalization. Xiaojun et al. (2004) add one more characteristic, which is accessibility, due to its power to be accessed anywhere, anytime. These characteristics apply to online transportation service as well which is one of varieties of m-commerce products. With its uniqueness, it is important to study the quality of service of online transportation.

Statement of the Problem
The researchers found out that the MDRRMO has no existing system for scheduling of request, dispatch and tracking of ambulance.
First, the employee of MDRRMO found it hard to schedule the request of an ambulance to be dispatch when accidents occur.
// problem/ cause// effect///
Second, the request is received via verbal most of the time that can cause delay in dispatching of ambulance.
Third, the employee of the MDRRMO can’t easily locate the ambulance that they dispatch.
Objectives of the Study
This proposal’s primary objective is to create an Online Request for Transportation and Management Service. Specifically, it aims to:

Review of Foreign or Local Literature
The overall objective of a literature review is to provide historical background to key concept and to provide a strong argument that guides the research problem. This section aims at creating a good system that supports the current study by reviewing a number or related previous studies that have dealt with online request transportation and management service.
Transportation request means a request for transportation services submitted to the operator in accordance with the booking manual. Transportation planning provides integrated solutions by balancing policy, investments, technology, and land use. This aim to support long- term growth and strategic accessibility. Successful transport planning analyzes community needs, travel patterns, and demographics before developing plans.
Importance of Online Request Transportation.
Review of Foreign or Local System
Related Systems is a part of a system documentation wherein, the researchers cite system/s they came across in which they draw some ideas of their proposed system. These are some of the systems related to the study.
Effect of the Online Transportation against Conventional Transportation and User of Transportation Services in Semarang Poncol Railway Station, Indonesia (Legal and Economic Perspective). One of the most effective and efficient public transport is the Ojek. Ojek or ojeg is an informal public transportation in Indonesia in the form of motorcycles. Called informal because its existence is not recognized by the government and there is no permission to operate it. Passengers are usually only one person. Ojek is widely used by residents in big cities because of its advantages with other transport, which is faster and can pass through the jam in the city. Usually a Ojek is available at a busy junction, or in the entrance to a residential area. Laws are not legally specified in Undang-Undang Nomor 22 Tahun 2009 tentang Lalu Lintas dan Angkutan Jalan, this Rules is expected to help organize and coordinate public transport and motorists on the road to be more orderly. This public transportation act as the carrier of people and / or goods to facilitate the community in doing their activities. Discussion on the development of the legal aspects of transportation cannot be separated from the effectiveness of the transport law itself. The more rapid technology of the era, making a lot of people use it to innovate, one of which is the use of Internet technology in the field of transportation. Indonesia has an online motorcycle service that allows prospective passengers not to come to an ojek base. Ojek online it booming began in 2014 because of easy access and cheap tariff given for
Using GPS-based technology to determine the tariff rather than the conventional motorcycle taxi that fix rates that are not in accordance with the distance traveled. The presence of ojek online is certainly triggered the reaction from various walks of life who rely on life with ojek service, the reaction will eventually lead to business conflicts between motorcycles conventional with ojek online.
The research method used in this research is the sociological juridical approach. The sociological juridical approach is to identify and conceptualize law as a real and functional social entity in the real life system. The sociological juridical approach is to emphasize research that aims to gain legal knowledge empirically by plunging directly into the object of knowing the positive and negative impacts of the emergence of online transport of conventional transportation at Semarang Poncol Station. The statute approach is conducted by reviewing all laws or regulations related to legal issues to be investigated, ie research contained in Law No. 22 of 2009 on traffic and road transport (Undang-Undang Nomor 22 Tahun 2009 tentang Lalu Lintas dan Angkutan Jalan).
