Introduction:
The purpose of this assignment is to focus on the care provided for a patient in clinical practice. Two key nursing problems will be highlighted and discussed in this paper. A holistic assessment of the patient’s health will be made using the biopsychosocial model (Engel, 1979). A care plan will be developed using goals that are Specific, Measurable, Attainable, Realistic, and Timely (SMART).
These patient goals will be in accordance with National Institute and Clinical Excellence guidelines relevant to their condition and associated nursing problems. The Nursing and Midwifery Council (NMC) professional standards will be applied (Nursing and Midwifery Council, 2018).
Peadar (Pseudonym used for confidentiality) was referred by his GP to the dementia assessment and treatment unit for a period of assessment, following concerns reported by Peadar’s family. The family reported biological and psychological symptoms of dementia (BPSD) which they were finding increasingly difficult to manage at home. Caring for someone with dementia can cause high rates of stress and psychological morbidity (Brodaty, 2009). Peadar has a diagnosis of Alzheimer’s Disease and type 1 diabetes.
According to the World Health Organisation 2017, (WHO 2017), Alzheimer’s Disease is the most common type of dementia, and may contribute to 60-70% of dementia cases. Of the 9 million people with Type 1 diabetes, the majority live in high-income countries Pan American Health Organization 2018 (PAHO 2018). The goal of assessment and treatment is to improve the patient’s quality of life, improve management of behaviors that challenge and stabilise blood sugars
