• Client Profile
• Client condition:
• Client’s needs
• Discuss the different services and care settings available for your client and the role of the MDT within these areas, including therapeutic interventions.
• Examine how you as a Carer would promote the following when caring for your client. E.g., empowerment, advocacy, etc.
• Reflecting on the care that you provide for your client, make recommendations on how to improve care practice to improve the quality of this care.
