ABOUT YOUR ASSESSMENTS |
This unit requires that you complete 4 assessment tasks. You are required to complete all tasks to demonstrate competency in this unit.
Assessment Task | About this task |
Assessment Task 1: Written questions | You must correctly answer all 16 questions to show that you understand the knowledge required of this unit. |
Assessment Task 2: Case study | This task requires you to read through a case study and then answer the questions about identifying and formulating development needs, strategies and goals. |
Assessment Task 3: Workplace project | For this task, you will need to complete a skills development plan for three clients with a disability. |
Assessment Task 4: Workplace observation | There are two parts to this task.
Part A requires you to implement a person-centred strategy and skills development for one client. Part B requires you to evaluate the client’s skills development and provide meaningful feedback based on client response and observation. |

How to submit your assessments
When you have completed each assessment task you will need to submit it to your assessor.
Instructions about submission can be found at the beginning of each assessment task.

At the beginning of each task in this booklet, you will find an Assessment Task Cover Sheet. Please fill it in for each task, making sure you sign the student declaration.
Your assessor will give you feedback about how well you went in each task, and will write this on the back of the Task Cover Sheet.
Make sure you photocopy your written activities before you submit them – your assessor will put the documents you submit into your student file. These will not be returned to you.

Case Study: Loretta |
Loretta is a 40-year-old female who experiences disability due to an acquired brain injury she sustained from a motor vehicle accident. Loretta uses a wheelchair, lives with her parents and is taken to a day centre on a regular basis to participate in social activities with staff support.
Loretta enjoys being involved with artwork such as painting as well as playing games with others. Some of her favourite games include playing cards and Connect 4. She gets along well with other clients at the day centre and looks forward to her day trips in particular riding in the car to and from the centre. Due to Loretta’s physical disability she has limited range of independent movement. She requires assistance with a sling and hoist when being physically transferred. Although she lacks the arm strength to help with mobility, Loretta has some range of movement in the thumb and forefinger of her right hand. Sometimes she can grasp light objects or material like a pen or light paintbrush using these fingers although it is quite challenging. When able to grasp onto a light object for a short period of time, Loretta gets excited and appears happy. When this happens she will often attempt to pick up and hold the item repeatedly. Being able to develop this fine motor skill would be beneficial in helping her with completing small tasks and activities more independently both at home and at the day centre. When she observes a demonstration on how to complete a task, she is often successful in replicating the steps required to complete the task. Slow movements and repetition seem to work well. |

Skills development planning should be undertaking during a meeting with the client, their family and/or carer.

Part B requires you to evaluate the client’s skills development and provide meaningful feedback based on client response and observation.

For this part of the task your assessor will observe you implementing a person-centred strategy for specific skills development as part of the client’s individualised plan.
You will be required to demonstrate the following:

Work with the person to implement ongoing skills development strategies.
For example, working with the client and their individual plan to ensure their needs and goals are being met
Work with the person in a manner that is respectful, motivating and empowering.
For example, collaborating with the client in a positive and effective manner at all times. Adjusting communication style to the communication style of the client, providing verbal encouragement and feedback before, during and after task completion.
Support and assist other members of the team to implement the strategy.
For example, ensuring that other team members are equipped with the information and resources needed to carry out the plan according to the needs of the client.
Use specified equipment and resources to facilitate the learning process.
For example, correct preparation and safe use of equipment, tools and material needed to implement the plan.
Document client outcomes.

For this part of the task your assessor will observe you evaluating a person-centred strategy for specific skills development as part of the client’s individualized plan.
You will be required to demonstrate the following:
Effective observation of the client for review of progress towards their goal
Provide valid feedback to the client, family and/or carer
Review the ongoing effectiveness of the plan
Modify the plan or learning strategy according to client feedback
Provide effective levels of encouragement and positive reinforcement

Monitor the client’s progress to ensure it is aligned with their needs and goals.
For example, observing the client perform tasks in line with their skills development plan and providing feedback to the client, family and/or carer to review the success or failure of client performance

For example, using the individual client plan and related documentation to review client progress and make changes to the plan accordingly.
Encourage learning and positive reinforcement.
For example, using all opportunities to encourage and support the client to learn or practice new or current skills, and reinforcing that behaviour.
