Through advocacy, comprehensive evaluation, planning, communication, health teaching and engagement, budget management, program facilitation, and the application of evidence-based standards or standards, case management assists clients in achieving wellness and autonomy. It aids the patient/client in achieving development as outlined in the plan of treatment. Education and information include assisting the patient/client and their family/support network in developing a greater grasp of key health and health care subjects (Fulmer et al., 2021).

Care coordination is purposefully planning patient care actions and exchanging knowledge among all people involved in a patient”s care in order to provide safer and more effective care. This implies that the patient”s requirements and desires are anticipated and conveyed to the relevant individuals at the appropriate time, and that this knowledge is used to deliver safe, appropriate, and efficient care to the patient. (Fulmer et al., 2021). The primary purpose of care coordination is to address the requirements and preferences of patients while providing high-quality, high-value medical care. This implies that the patient”s requirements and preferences are understood and conveyed to the relevant individuals at the right time, and that this information is utilized to guide the administration of safe, appropriate, and successful care (Fulmer et al., 2021).

The system of services and supports offered to people with intellectual and developmental disabilities (I/DD) appears to be stuck in time. Home- and community-based options are quickly replacing the previous system of institutionalized care. Simultaneously, financial restrictions and onerous rules limit the provision of supportive housing for the I/DD population. Similarly, the old medical paradigm of treatment is making way to person-centered living plans that reflect persons with I/DD”s objectives and skills. However, there is no mechanism in place to coordinate and implement those plans in a system currently dominated by fee-for-service providers (Heller, 2019).
