Assessment Due:

You are caring for a client who is living with Crohn’s disease.

Research the condition, and using the template provided below, respond to the following points;

Identify the condition and highlight the causes.

Identify the most common signs and symptoms (at least three) and discuss the monitoring processes

Discuss the signs that would indicate the client’s condition is deteriorating.

From the perspective of a Healthcare Assistant and with consideration of the client’s complex needs, describe and evaluate at least three practical and supportive care interventions you can provide as a Healthcare Assistant.

The health condition is discussed in detail with reference to relevant literature.

At least three symptoms are identified and are clearly linked to the health condition.

Monitoring processes demonstrate a broad knowledge of the health condition and link to the identified symptoms

Signs of deterioration are discussed in relation to the health conditions and link to the expected disease progression

Care interventions are evaluated in relation to the client’s presenting complex needs and link to the role of the Healthcare Assistant.

Merit Criteria

The relevant health condition is outlined with identified causes discussed.

At least three symptoms are identified with consideration given to the associated health condition.

Monitoring processes are identified and discussed in relation to the identified symptoms.

Signs of deterioration are discussed and outlines the expected progression of the disease.

Care interventions are discussed and demonstrate knowledge of the role of the Healthcare Assistant in relation to providing care to the client with complex health needs. 

Pass Criteria

The health condition is defined and causes are highlighted

At least three symptoms are identified with a discussion of monitoring processes for each

Signs of deterioration are discussed

Care interventions are described in consideration of the complex needs of the client from the perspective of the Healthcare Assistant

Fail Criteria

The health condition is not defined. Causes are not identified.

Three symptoms were not identified. There was no discussion of monitoring processes for each symptom.

There was no discussion on the signs of deterioration.

The complex needs of the client were not taken into consideration.

Care needs were not identified in relation to the role of the Healthcare Assistant.
